Participating states establish and use the required health-sector coordination structures, plans and budgets to prepare for implementation.
Priority primary healthcare facilities and relevant emergency maternal and newborn care facilities are identified and strengthened to improve readiness, staffing and service capacity.
Strengthened health facilities and community-based services deliver the essential primary healthcare and maternal, newborn, child and adolescent health services supported by the programme.
Programme financing and technical support are channelled through the existing health-system structures and mechanisms to enable states and facilities to implement eligible interventions and achieve programme results.
Implementation and results are monitored against the programme's indicators, with verification and reporting used to assess progress in service quality, utilisation and health-system resilience.
Key figures from official programme reporting, shown with their reporting dates and evidence basis so that scale is not confused with independently measured impact.
Federal health-sector reporting in July 2026 associated the HOPE-PHC/NHSRII implementation framework with reported progress in primary healthcare revitalisation, maternal and newborn services, frontline health-worker training and increased use of primary healthcare services. The Federal Ministry reported 3,026 PHC facilities revitalised, 78,054 frontline healthcare workers trained, and 42,970 maternal and neonatal CEmONC services delivered between October 2024 and March 2026. These figures are government-reported health-sector results and should not automatically be interpreted as outcomes attributable solely to HOPE-PHC.
The World Bank's programme documentation provides a formal results framework, implementation monitoring and defined indicators for HOPE-PHC, including service readiness, utilisation of essential services, maternal and neonatal care, digital health and health-system resilience. However, the sources reviewed do not yet provide a comprehensive independent impact evaluation isolating HOPE-PHC's effect on mortality, household health expenditure, income or long-term health outcomes.

What the Record Establishes
Clear answers about the programme, eligibility, delivery, application process and how to access support safely.
HOPE-PHC is the Primary Healthcare Provision Strengthening Program, a federal results-based programme designed to improve the quality and utilisation of essential primary healthcare services and strengthen Nigeria's health-system resilience.
The programme is intended to benefit people who depend on Nigeria's primary healthcare system, with particular attention to women, children, adolescents and vulnerable populations requiring essential health, reproductive, maternal, newborn and nutrition services.
HOPE-PHC is not presented as a general individual application or cash-support programme. It works through Nigeria's federal and state health systems to strengthen healthcare facilities, services, financing, workforce and community-level delivery.
The programme works through existing federal and state health structures, including primary healthcare facilities, community-level services and health-system mechanisms that support essential services and referrals.
The programme focuses on essential healthcare services, including reproductive, maternal, newborn, child and adolescent health and nutrition services, while also supporting primary healthcare quality, utilisation and health-system resilience.
Patients do not normally access HOPE-PHC through a separate application portal. Healthcare access is through participating primary healthcare and other health facilities within the state health system. The Federal Ministry of Health and Social Welfare also provides a Facility Finder to help people locate healthcare facilities.
The programme has defined results and implementation indicators, but it remains under implementation. Programme financing, activities and reported outputs should not be confused with final evidence of long-term effects on mortality, household health costs or other population health outcomes.
